Contribution of Percutaneous Drainage to Postoperative Intra-abdominal Abscesses


Abstract
Intra-abdominal abscess or fluid collection is not common complication after abdominal surgery But, if this complication develop, it is troublesome to treat and sometimes lethal for patients. To eveluate the causative operation and the treatment
result
of percutaneous drainage, we analysed 26 patients who intra-abdominal abscess was developed after abdominal surgery during 5 years from January, 1986 to December, 1990. In the same period, we had done 3171 abdominal operation.
1) Male to female ratio was 1 : 1 and the most common age group was sixth decade(23.)%).
2) The common previous operations of the intraabdominal abscess were hepatobiliary operation(46.1%), appendectomy(26.9%), and gastroduodenal operation(15.3%), But the development ratio of intraabdominal abscess to total operation number of each
organ
were pancreatic operation(7.1%), hepatobiliary operation(2.2%) and gastroduodenal operation(1.5%) in turn.
3) The common symptoms and signs were fever and chill(92.3%), localized tenderness(80.7%) and abdominal pain(76.95%).
4) The leukocytosis was 84.6% and abnormality of LFT was about 20%.
5) The main diagnostic method was ultrasonography and computed tomography and sensitivity rate 91.3%, 100% in each.
6) The common locations of abscess were subphrenic space(34.5%), liver parenchyme(24.1%) and inter-loopal space(20.7%).
7) Positive culture was obtained in 17 of 26 cases(65.4%) and E-coli was the most common organism(41.2%).
8) The percutaneous drainage was successful in 20 patients(77%) and the average duration of drainage was 22 days
9) Over-all morality was 3.8% and the cause of death was sepsis.
